COVID Waiver

Since we are gathering in a group and everyone has their own personal feelings and beliefs about COVID viruses, please be mindful and respectful of others' wishes and desires while attending the retreat.
Additional Waiver, Release, and Responsibilities
This waiver, release, and acknowledgement of responsibilities is unfortunately necessary during this unique time of the COVID VIRUSES pandemic. By signing up for and/or attending my retreat or training, you agree to the terms of this document.
You agree that you will not attend the retreat if you have any symptoms of COVID VIRUSES prior to the departure for this trip. These symptoms include fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion, runny nose, nausea, vomiting, or diarrhea.
COVID VIRUS CONSENT FORM (by signing this form, you agree to and acknowledge all of the following).
I confirm that I am not presenting any of the following symptoms of COVID VIRUSES listed below:
• Temperature above 100 degrees • Shortness of breath • Loss of sense of taste or smell • Dry cough • Sore Throat • Chills • Fatigue • Muscle or Body Aches • Headache • Congestion • Runny Nose • Nausea • Vomiting • Diarrhea
🌟 I confirm that I have not knowingly been around anyone with these symptoms in the past 5 days.
🌟 I do not live with anyone who is sick or quarantined.
🌟 I understand that travel increases my risk of contracting and transmitting COVID VIRUSES.
Symptoms During the Retreat:
In the event you develop any symptoms during the retreat, you must notify LG immediately. It is understood that this may result in you incurring expenses associated with separate accommodations and travel; however, you understand these expenses are not the responsibility of LG and must be incurred by you. You consent to cooperating with LG's request to leave at her sole discretion and to being completely honest with LG about any symptoms you may experience. As provided below, this is why LG has a mandatory Travel Insurance Policy to cover these and other expenses prior to departure.
🌟 I agree to inform LG immediately in the event that I develop any symptoms during the retreat.
🌟 I understand and agree that I am fully responsible for any expenses incurred or associated with separate accommodation and travel; this is not the responsibility of LG.
🌟 I understand and agree to be fully honest with LG regarding any symptoms I may develop and agree to comply with a request by LG to depart as determined by LG in her sole discretion.
Waiver of Coronavirus-Related Illness Indemnification: You understand that you may be exposed to the COVID VIRUSES by another retreat attendee and/or an individual to whom you may be exposed while traveling to, from, or after the retreat. You agree to waive and release LG from any liabilities, ties, or damages you may incur in the event you contract COVID VIRUSES.
🌟 I agree to waive and release LG from any claims, liabilities, or damages I may incur in the event I contract COVID VIRUSES.
Cancellation: In the unfortunate event you need to cancel your trip, cannot attend due to you or someone you live with, or someone you came in contact with contracting COVID VIRUSES or developing symptoms prior to the retreat, or must leave the retreat for any reason, LG cannot provide a refund.

ACKNOWLEDGEMENT
I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators, and representatives.
Additional Waiver. This waiver is incorporated into and made a part of other waivers you agreed to in connection with signing up for the retreat.
